Welcome to the application page for the 2024 Profile for Success (PFS) Program. To apply to the 2024 PFS Program:
  • Please make sure that you save your application after each session.
  • Prior to submitting, make sure that your application is complete and you have uploaded:
    • A 2-3 paragraph essay explaining how you qualify for PFS
    • A list of the Biology, Chemistry, Physics, Math, English, Sociology and Psychology course (s) you have taken and/or are currently enrolled
    • A DAT Report (if taken)
    • A 500 word minimum essay that addresses the short essay questions
    • A current resume or curriculum vitae
  • Do not forget the following information is required for your application to be complete:
    • Three letters of recommendation along with the applicant recommendation form (from each recommender). Two letters should be from science instructors/ professors and the third can be from an individual of your choice. Please complete the contact information in the Recommendation Section within the application. Emails will automatically be sent out to your recommenders with the link to the Recommendation Form.
    • Official college transcripts from each institution you have attended.
      Transcripts should be sent electronically via a transcript service (eg: www.parchment.com or Credentials TranscriptsNetwork, etc.). The school registrar may also send electronic transcripts directly to the email below. Mailed transcripts must be received by March 1, 2024.

Program Qualifications

Please supply the following information. Check any/all appropriate boxes that would describe yourself by using the guidelines to determine your eligibility. Please upload a corresponding essay (2-3 paragraphs) describing how you qualify for PFS. Only one category is necessary to qualify. Documentation is not required at time of application but must be available upon request.

2 Semesters of Biology with Labs
2 Semesters of Inorganic Chemistry with Labs
2 Semesters of Organic Chemistry with Labs
1 Semester Physics (recommended but not required)
1 Semester of College Level Math
1 Semester of English Composition

Please be advised that all program pre-requisites must be fulfilled prior to the program in order to qualify for admission into PFS.

Personal Essay Questions:

Please provide a short essay in which you introduce yourself, and address the following questions:
  1. What exposure have you had to the field of dentistry and how has this influenced you?
  2. What are your goals as a health professional?
  3. How would you describe yourself? How would others describe you?
  4. Explain why you want to participate in this program and why we should select you as a participant.
  5. What unique skills, qualities or life experiences would you bring to a health profession?
Upload your typed essay. Please save an electronic version of your answers to be used if you are accepted into the program.

Recommendations:

Three (3) letters of recommendation should be submitted directly from each person making the recommendation. Note: Two letters should come from your science instructors and the third may come from an advisor/counselor/employer or person of your choice.

List names and titles of the people you have asked to complete the recommendation form received with this application. YOUR RECOMMENDATIONS SHOULD INCLUDE AT LEAST TWO SCIENCE INSTRUCTORS.
